Upgrade to v1.4.1 left the "Are you secure?" block!

This block doesn't work now because phpblock_getBent is history. Just tell me what happens in new installations and I'd do it too. That is, do they just not install this block anymore? What about that new sectest.php - is it run through moderation.php instead?

Yeah, the code that was in the getBent block is now part of sectest.php. Which is linked from moderation.php until all tests pass.
The install script should probably have removed the old getBent block when upgrading. This simply didn't occur to us since I for example didn't have it any more on any of my sites ...
