Forum Plug-In: cannot add a category

My clean install of the Forum plug-in went fine, however I cannot seem to add a category to the forum. I get a "Page cannot be displayed" error message when I click on "Add a Category". I also get the same error when I try to update the Forum settings.

I've checked my folder and file permissions (folders have read and execute) and files have (read) and cannot figure it out.

Any thoughts would be greatly appreciated!

What version of the forum?
Is this using the supplied language file?
