I'm not sure I understand what your problem is ... If you create a "normal block" you can stick all the HTML you want into it, including external links. PHP blocks are created using some lines of PHP code which again can include external links. And portal blocks do have external links by definition ...

The easiest way to get a link into the menu bar is to hard-code it into your header.thtml file - nothing magic about that, either.
