Looking for script help!!!!!!!

Here is what I need.

How much to write this script?

and lastly, how much to implement some kind of tracking/sponsor/affiliate program?


FIRST PROJECT: First we want to build the survey form I want so I can put a link in my email response

SURVEY/FEEDBACK

I wants to ask for feed back from customers by putting an active link in an email follow up message. Clicking the link will go directly to a form on the web that will collect the information. Filling out the form will submit the information to the Geeklog Forum Testimonials.



FORM FIELDS (info to collect)
Name

Phone

Email

Text box for comments or testimonials.



FORM RESULTS.
Values entered into the form fields will be returned to the Geeklog Forum, into the Testimonial Section, with an email notification copy going to the specified email address.



ADMINISTRATOR VALIDATION

The comments will not be published in the testimonial section until approved or validated by the administrator.



PUBLISHED RESULTS

After administrator validation, the testimonial or feedback will appear in the Testimonial box on the front page, and will show the Name submitted as the username in the post.

If I'm reading this correctly, the actual servey script would be pretty straight forward.
Posting into a forum from the script should also be pretty easy to do.

What might not be straight forward would be the changes to the forum.
You're going to need to run somekind of moderated forum, where moderated means all comments get added to a submissions queue before being added to the site. I don't know of a forum that provides that - one may exist but I've never looked.
You also mention having the testimonial listed on the front page - the geeklog forum doesn't have that ability yet. I see no reason why you couldn't pull the information from the forum tables into a static php page though.

Or are you not actually talking about a forum, but rather a topic section ?

If it's a topic section, then it shouldn't be too bad to do.

I'm not sure that I see a need for this to be driven by the forum engine, the topic engine should be able to offer what you want.
