prediction plugin or block

I'm looking for a plugin (or block) where registered members of my site fill in a prediction form with what they think are gonna be the soccer results for that weekend.
The form also has te calculate the results of every member en save this in a table where the can see there own standings.
ANyone who can help me with this or known a plugin/block?
