How can I add a 'Contact' field to the FILEMGMT submission page?

i want to allow 'anonymous' submissions...but i want a 'contact' to be visible to the admin.

Any ideas how this might be done? Even if it just requires having an email sent to the admin with the contact details. I do not want the contact details to be visible when browsing 'downloads'. I've tried playing around with different things, but i run into mySQL errors.
