Forms tool for Geeklog?

Before I start reinventing the wheel, I thought I should ask; is there a reccomended tool which makes it easy to create forms that users can fill in and submit in Geeklog?
The forms should have these characteristics;
1) The user fills in the form an presses "OK" (or "submit" or ...)
2) The filled-in form is presented (or a web-page with the results from the form) with a
"confirm" button (and perhaps a "restart" button as well).
3) When the user has confirmed, the data from the form (filed names + data) is written
to a .csv file and mailed to the recipients.
I have looked at phpformgen, I can probably modify a form created with it to do what I want if no better tool exists.

Well, not related to Geeklog but check out PEAR::HTML_QuickForm. It's an easy way to generate forms dynamically.
