BUG: Plugin icons overwriting each other

Hi all,
Has anyone else run across this, and come up with a fix?
It seems that when I go into the moderation.php/plugin admin, the last installed plugin has over wrote the two previous plugins. So now that I have three plugins installed, the last plugin, forum, is now listed three times, and the previous plugs are no longer listed.
I noticed this when I installed the second plugin (Menu), and didn't think anythng of it. But, now that I installed forum, I no longer have access thru this page to the admin sections of the prior to plugins (staticpages & menu).
I also noticed that on my Admin Only Block there are three blank (N/A)'s listed below plugins toward the bottom of the list.
I can go into the Plugin's page via Admin Only block, and all plugs are listed. All plugs are still working, I just can't access the admin porition of the plug through the moderation.php page.
Has anyone seen this behavior before? any suggestions to correcting it?
I'm running:
Geeklog 1.3.9sr1
Apache 1.3.31/PHP 5.0.1
FreeBSD 5.2.1-RELEASE

So, is this a known issue with PHP5?
I upgraded to PHP5 when PHP4 there was an apparent bug in PHP4 that was not allowing RSS/XML feeds to work properly. I upgraded to PHP5, and the feeds began to work.
So is this a problem with PHP5?
