I replied to Andrew's email with the following, and am posting it here with the hope that it will help someone as well. There are also a couple of quick additions at the bottom to tighten up the theme a little. Soon I will be releasing an updated version.

Unfortunately in this situation, Refresh is a fixed width theme. Basically, the <div> tags that create the left block container, and the middle/right story container have their width set in the style.css file as percentages (adding up to 100%). Geeklog will essentially skip over the content of leftblocks.thtml when staticpages or plugins are configured to not show left blocks. So, I can get it to not render the left blocks, but it just leaves a white space as the <div> that creates the story area is set to float right and has a width of 72% or something.

Geeklog achieves the story column filling the space by using 100% width tables for the flexible story middle column, but since Refresh isn't a 100% width theme (ie. it doesn't fill the browser window at higher resolutions) but rather a fixed width theme, it's like trying to fit an apple into an orange and end up with something pretty. :-(

In looking at your site, it doesn't look like the phpbb forum spills out much on the right. I would think that if you just edited the header background graphic, the side wrap graphic, and the footer graphic and stretched them out a bit, it might do the trick. The three files to edit are header.jpg, content.jpg, and footer.jpg. Then, in style.css, you will want to change the width values respectively for the following selectors:
#wrap
#content-wrap
#header
#header h1#logo-text
#header h2#slogan
#footer

ADDITIONS:
To fix the login box, simply remove the display:block; property and value from the label selector in style.css like this:

margin:5px 0;
}
